National Center on Advancing Person-Centered Practices and Systems

The National Center on Advancing Person-Centered Practices and Systems (NCAPPS) is a new initiative created by the Administration for Community Living and the Centers for Medicare & Medicaid Services that seeks to administer person-centered planning in states, territories, and tribes. The NCAPPS has a mission of enacting person-centered planning for those in need of support and services and making it more of a reality.
