Core features include home-ownership, a common house and amenities, weekly common meals, resident-driven management, and pedestrian-oriented design. Live-Work-Play Community - A planned community that may include social enterprises, employment opportunities, and recreational options on the housing property. Planned Community - A multi-family, intentionally-planned community. Planned Community with Built-in Support Services - Rent may include an on-site concierge, smart home technology, planned activities, or staff to provide additional supports and amenities. Single-Family Home Townhouse This will be used in a filter to describe the kind of property type, not how supports and services are delivered.Support & Service Delivery Model Adult Foster Care / Family Host Home - You are invited to live in someone’s home. That person will provide natural supports. The host-family can request for you to move. Consumer-Owned / Controlled Setting - Your services and supports are self-directed, and you are responsible for managing your housing. Intentional Community - Individuals in the community desire to be accessible to each other and provide natural supports to their neighbors. Provider-Owned / Controlled Setting - The service provider may also be your landlord or at least manage the property. Shared Living / Roommates - You have at least equal authority over the home you share with roommates without disabilities who may also provide natural supports. You can ask them to move. Payment Option Affordable Housing Units - A portion of units in a housing development with fixed-rent. Designed to be affordable for those with low or very low income in that community. Home and Community-Based Services Waiver - Each state has its own set of waivers, but they all offer Medicaid funding for support services outside of institutional settings. These offer permanent rental subsidies in which the resident pays 30% of their total income. Intermediate Care Facility (ICF) - The setting and supports are publicly funded and combined in an ICF. Private Pay - Support staff and services can be paid for by the resident, their family, or a special needs trust. This information will help users filter for financial accessibility.Lifestyle Agricultural / Ranching Community - Animal care and/or tending of crops center the rhythm of life. This lifestyle can be found in urban, suburban, or rural settings. Faith-Based - Lifestyle centered around a specific faith-community. May incorporate spiritual rituals into daily life and encourage religious affiliation. Health / Fitness-Oriented - Common emphasis on making healthy food choices and prioritizing exercise. Live-Work-Play Community - Recreational and employment opportunities are within walking distance of the housing property. Suburban - Slower pace than an urban setting, access to transportation is an important consideration when living in this environment. Transit-Oriented / Urban - Densely populated and bustling with activity, this environment offers easy access to public transportation and/or sidewalks. Transitional / Collegiate - A 1-5 year program targeted to individuals who desire to live independently; not a permanent home setting. This information can be used to filter for the type of lifestyle one may prefer.Level of Support 1:1 Support - Full attention and support of at least one direct support provider is needed at all times. 24/7 Support - Support must be accessible in person at all times but does not need to be 1:1. Behavioral Support Daily Medical Support - Need medically trained and/or certified support staff to safely complete daily routines including feeding, breathing, transportation, transfers, etc. High Behavioral Support - Past aggression to one’s self or others has prevented this person from fully accessing their community. Structured planning is necessary for this person to participate in community life safely. Moderate Support Needs - Support is needed throughout the day, but one can be self-sufficient without in-person support for hours at a time. Semi-Independent / Drop-In Support - Direct support staff is unnecessary for the majority of the day, or an an in-person check-in every few days is sufficient support. Primary Residents Autistic Adults - Home is centered around the needs of those on the autism spectrum. Individuals with I/DD - Individuals with a range of intellectual or developmental disabilities are the primary residents of the home. Neurodiverse - Individuals with and without disabilities live in the same home and/or planned community. Availability In Development - Group or individual has identified property for a residential setting, and housing will be available within two years. Now Planning Stages - Group or individual in the process of organizing and coordinating a new housing opportunity. May or may not be an established entity yet.
